Information for new patients

 

Introduction to Dr Vasquez:  Dr Alex Vasquez is an expert clinician, researcher, author and international lecturer who cares for patients in his office and educates other physicians world-wide on the best means to improve health outcomes.  Dr Vasquez holds 3 doctoral degrees from fully-accredited American universities.  In clinical practice, he treats a wide range of conditions and has particular interest in inflammatory, (auto)immune conditions such as:

• lupus,
• rheumatoid arthritis,
• ankylosing spondylitis,
• chronic fatigue, fibromyalgia, chronic pain,
• psoriasis, and
• eczema.

Metabolic disorders such as hypertension, hypothyroidism, diabetes mellitus type-2, and fibromyalgia are --in the vast majority of cases --  very easily treatable conditions.

 

Visits can be made on a selective basis in person at the consulting office in Portland Oregon. Additionally or optionally, telemedicine consultations can be held via phone or Skype per details below:

4 steps to have your consultation: 

1. Call or email Dr Vasquez to discuss your condition: There is no charge for this brief phone/email consultation:

2. Download and complete "new patient intake forms": These forms should be completed and present at your visit so that we can make fast and efficient progress. If you don't have these ready at the time of visit, then we will get off to an unnecessarily slow start that will impair your care and my ability to get the data I need.
   • You can print these and bring them to your visit.
   • You can write and mark on the PDF document by using a program such as FoxIt (for PC computers)--I use this program all the time for marking and highlighting documents.  It is free and available at  www.foxitsoftware.com
   • Whether you print on paper or edit and send the PDF document, it would be a wise choice to bring a printed paper copy to your visit to ensure that we have everything we need to make progress and are as free as possible from logistical hindrances.
3. Bring any/all prior lab results with you: Please bring copies to leave at the office. To request in advance that your prior records are sent from your other doctor(s) to Dr Vasquez, send this Records Request Form to your other doctors' office.
4. Pay for 1-hour, 90-minute or 2-hour consult: Use the link below to pay and confirm your appointment.  So that you can be called to arrange your consultation, please provide your phone number (email is automatically included) in your information when securing your consultation.
   • Most new visits take at least 90 minutes of interaction with generally at least 30 additional minutes of paperwork, prescriptions (if needed), and some follow-up time for questions and answers; therefore, purchasing 2 one-hour increments is most appropriate for most new patients. Half-hour increments can be purchased here.

Phone Consultations and Skype Visits: These are scheduled after receipt of your payment (click "Order Now" below).

Consultations & Telemedicine: Dr Vasquez travels extensively and works in several locations. For this reason, most consultations occur over the phone and via Skype. Dr Vasquez also has many out-of-state and out-of-the-country patients and the phone/Skype format allows these patients to receive care without having to travel for every visit.

Skype: http://www.skype.com/ is a free internet-based program that allows virtual meetings via an internet connection. Dr Vasquez's username will be sent to you following the scheduling of your appointment and receipt of your payment.

 Professional consultations—scheduling and pre-payment

Dr. Vasquez frequently receives requests for consultations from patients, doctors and other healthcare providers seeking advice and guidance on various health problems.  Dr. Vasquez draws on his extensive education, post-graduate education, and personal library of thousands of research abstracts and paper/PDF articles.

First consultations are almost always at least one hour. Included with this one-hour consultation are the following:

• History
• Physical exam (for in-person consultations)
• Review of previous lab tests and imaging reports
• Development of new treatment plan for your condition(s)
• Some additional follow-up and paperwork after the visit provided this is not excessive.

Consultations—whether in person or over the phone or internet—must be pre-paid and can be purchased by clicking the "order now" button to the right-hand side of the screen from this paragraph. Once your payment is received and processed, you will receive an email or phone call to schedule the exact time and logistics/location of your consultation. (If shipping charges are added automatically, these will be removed at time of processing. No shipping charges are applied to consultations.)

Dr Vasquez does not bill insurance and does not accept Medicare/Medicade. Payment is made with cash, check, or credit card. A detailed receipt with appropriate billing codes can be provided upon request to submit for insurance reimbursement.

Phone/email/Skype/office consults and office hours are by appointment only.

Cancellation policy: All appointments must be pre-paid in order to ensure that we are both committed to the process and ready to move forward. Cancellations by phone or email with more than 24 hours notice are given a 50% refund. Cancellations within 24 hours provide no opportunity for us to reschedule another patient within that timeframe and are thus nonrefundable. If you are not ready to commit to an appointment, please visit this website later when you are ready to make the committment and move forward.

New patients should print and complete the "new patient intake forms" prior to the consultation time.  Be sure to bring these to your appointment; if you are able to scan and send by email, please do so since all of your records must be saved as digital documents.

Please note: These forms are currently being updated from Dr Vasquez's previous practice in Texas. Please complete the available forms as they are now while the updates are being made to reflect Dr Vasquez's new practice under his naturopathic license in Oregon. Out-of-town and out-of-state patients often choose to fly to Portland for visits with Dr Vasquez.  Portland International Airportort (airport code: PDX) is an excellent, beautiful, and well-designed airport that is well-connected nationally and internationally with most major airlines. The airport is easy to access by rental car, bike, and Portland's excellent rail transportation.
