Psoriasis and bystander activation—Keratinocytes are immunologically active cells that present bacterial-derived superantigens to T lymphocytes. (J Dermatol Sci 1993 Oct[1]): Most doctors and researchers have believed that epidermal keratinocytes were relatively passive components of the outer layers of the skin where these cells form a mechanical barrier against injury and infection; likewise, their role in skin diseases was considered passive, with keratinocytes acting as passive targets and “innocent bystanders.” However, newer research mandates that we reclassify keratinocytes from passive barrier cells to “fully fledged members of the immune system (i.e. immunocytes)” based on the findings that keratinocytes produce important cytokines, adhesion molecules, and mononuclear cell chemotactic factors. Activated keratinocytes “can initiate and perpetuate the inflammatory and immunological reactions in the skin which contribute to the pathobiology of psoriasis.” Specifically, cytokine-activated keratinocytes can present bacterial superantigens to T-cells, resulting in activation and proliferation of these effectors of psoriasis.
