Skin disorders and superantigens—Superantigens in dermatologic diseases. (Dermatology Online Journal 2008 Feb[1]): In this excellent review of molecular biology and clinical disorders, the authors note that microbial superantigens are associated with many important clinical disease that affect the skin, including staphylococcal toxic shock syndrome, staphylococcal scalded skin syndrome, guttate psoriasis, psoriasis vulgaris (chronic plaque psoriasis), Kawasaki syndrome, atopic dermatitis, cutaneous T-cell lymphoma, and acute juvenile pityriasis rubra pilaris. The authors state, “Evidence shows that T-cells in psoriasis are triggered by conventional antigens and superantigens. It is suggested that although the process is initiated by bacterial superantigens, molecular mimicry between the bacterial antigens and keratin 17 leads to activation of autoreactive T-cells and persistence of disease.” The authors note that all streptococci secrete SPEC, a superantigen known to stimulate marked expansion of T-cells which display a particular marker “Vβ2”; these Vβ2 T-cells are found in increased quantities in acute skin lesions of patients with guttate psoriasis.
